Randomize a run

Updated September 2026

Press Generate on this Isaac item randomizer for a random item from The Binding of Isaac (TBOI): a game-warping Brimstone, a humble Sad Onion, a Yum Heart, a Curved Horn trinket.

Isaac is about the items you stumble into, so rolling them is half the fun and most of the theorycrafting. Pull three passives and picture how they stack: does Brimstone alongside Polyphemus stay a screen-clearing laser, or do the modifiers fight each other? Some pairings are famous, like the flies a Guppy set builds up, and half the joy is spotting a sleeper combo before you have to play it. Set a build-around challenge where everyone starts forced to make the same rolled item work, or settle an argument about which item is stronger by rolling both.

The filter matches what you want to think about: passives for a stacking puzzle, actives for an on-demand effect you plan a charge around, or trinkets for the small modifiers that change a run at the edges. Leave it on All for the full grab-bag the game throws at you.

Spin the wheel to land one pick live on stream. It is a natural fit for community races: let the draw decide a run's gimmick, or hand everyone the same starting item and see who reads it best. Share a link that carries your filter and count so everyone theorycrafts from the same draw.

The names are real items from the game, text only, so every roll is something you could genuinely find on a run.

Frequently asked questions

What does each item do?

Each result shows a one-line summary of its effect, and the list under the generator carries the same line for every item. The summaries are ours, written for players who want the gist before they pick an item up.

How do I run a build-around challenge?

Roll one item and have everyone start a run forced to build around it, or roll a few passives and theorycraft how the synergies would stack.

Can it randomize a whole run?

Yes. Randomize a run under the generator picks a character, a boss to beat, a difficulty and one rule, and you can swap any one of them.

Can I filter to active, passive, or trinket items?

Yes. The filter narrows the roll to passive items, active items, or trinkets.

Does it include Repentance items?

Yes. The pool holds 892 items and trinkets through Repentance+, Spindown Dice, Abyss and Flip among them. Quest pieces like the Key Pieces and Dad's Note are left out, along with the few items no pool can drop.

Is this the same as an Isaac item randomizer?

Yes. Press Generate for one random item, or roll a handful to randomize a full set for a run.

The full pool, and what each item does

Everything a roll can land on, split into passives, actives and trinkets.

Every item and trinket through Repentance+, minus quest pieces and the two pocket items only one tainted character can hold.
